How to enter a new client & pet into ezyVet who WAS adopted through OHS

  1. Search for adopter in SB with phone number, name, or email address.

  1. Use the adopter’s person ID number for their EV profile.
  2. Select “List All Animals”.

  1. Select “Edit” to view appropriate pet.

  1. Use pet’s AID number for EV profile.
  2. Select “View Medical History”.

  1. Select “Email”.
  2. Enter [email protected] in the email address field.
  3. Send.

  1. Enter adopter’s PID number prefaced by “OHS_” as their EV PID under “Code”. This makes it easier for all departments to cross-reference.
  2. Verify and copy phone number, email, and home address from SB.
  3. Save.
  4. Select “New Profile”.

  1. Search for pet using SB PID number and selecting “All” under search field. If pet does not appear, enter all data manually from their SB profile.
  2. If pet does not appear under the search field, click on pet to auto-load their SB information.
  3. Update “Owner” with adopter’s SB PID number (it should auto-load with just the digits).
  4. Update pet’s name (remove any former shelter names).
  5. Update pet’s code to SB PID prefaced by “OHS_”. This makes it easier for all departments to cross-reference.
  6. Select “Attachments.”

  1. Open ohsinfo email from SB and drag medical record into profile.
  2. Save.
