• Doctor request (check communications, memos, and/or SOAP notes)
• Follow up on an Urgent Care or Sick Pet exam within 30 days

THIS TYPE OF APPOINTMENT SHOULD NOT BE USED FOR:

• Patient has new presenting problem not included in original exam (schedule Sick Pet or Urgent Care exam)
• Patient needs suture removal (schedule Tech appointment)
• Patient has not been seen at CVH for the same problem within 30 days (schedule Sick Pet or Urgent Care exam)
