THIS TYPE OF APPOINTMENT SHOULD BE USED FOR:

• Exam and boosters for puppies and kittens UNDER 5 MONTHS OF AGE who are already established patients
• Kittens and puppies must be under 5 months of age at the time of the appointment

THIS TYPE OF APPOINTEMNT SHOULD NOT BE USED FOR:

• Puppies and kittens who are OVER 5 MONTHS OF AGE
• Pets who have not already established care through CVH (schedule Wellness, Sick Pet, or Urgent Care exam as appropriate)
• Patient has new, presenting problem (schedule Sick Pet or Urgent Care exam as appropriate)
• Patient needs anything other than vaccines
• Patient has only been seen through Spay & Save
